Telehealth Consent
Last updated: October 1, 2026
Nature of telehealth
Telehealth uses electronic communications and technology to allow you and a licensed provider to interact when you are in different locations. It may include questionnaires, secure messaging, audio or video encounters, electronic prescribing, care coordination, and review of information you submit.
Benefits and risks
Potential benefits include convenient access and timely care. Risks include technology failure, delays, incomplete information, privacy or security incidents, limitations on examination, and the possibility that telehealth is not appropriate for your condition. A provider may require in-person care or additional evaluation.
Your responsibilities
You agree to provide accurate and complete information, disclose relevant conditions, allergies, medications, pregnancy or breastfeeding status, and changes in health, follow provider instructions, ask questions, and seek in-person or emergency care when directed.
No guarantee of treatment
A provider will use independent professional judgment. Completing an intake does not guarantee diagnosis, treatment, or a prescription. You may withdraw consent to telehealth at any time, but withdrawal does not affect care already provided or records already created.
Privacy and emergencies
Telehealth information is handled under applicable privacy laws and the Privacy Policy. Telehealth is not an emergency service. Call 911 or go to the nearest emergency department for urgent or life-threatening symptoms.
Consent
By checking the telehealth consent box and continuing, you acknowledge that you understand the nature, benefits, risks, alternatives, and limits of telehealth and voluntarily consent to receive care using telehealth technologies.
